Step 1:

Create an 800x50 document.

Step 2:

Rename Layer 1 to Background and fill it with #48b911. 

Then, add the following filters to it:

Step 3:

Create a new later. Using your Rectangular Marquee Tool, select about 50% of the bar. Then select your gradient tool and choose the Foreground to transparent option. With the new selection in place, drag from the top to bottom.

Step 4: 

The next step is to add some text. I used to font jethose PIXEL for this. No special text effects necessary, unless you want them.

Step 5: 

Now here comes the fun part, the dividers! Create a new layer and zoom into somewhere around 1600% and select the Rectangular Marquee Tool. Select a box that is 1 pixel wide, but 1 pixel away from each end, top and bottom.

Now fill the selection in with #3E3E3E. Now select the same kind of box, except its one pixel over to the left. Now fill that with #FFFFFF. You should now have this:

Step 6:

Now apply the following layer style to it:
